How Kehai Care decides

Rules as of 30 September 2026 · 36 rules · 53 symptoms

If this is an emergency, call 911. This page explains how the app reaches an answer. It is not a way to check your own symptoms, and it is not medical advice.

How the rules are used

You describe how you feel. An on-device model picks out symptoms from a fixed list of 53, only ones grounded in words you actually said, and shows them to you to confirm or correct. Then the rules below run. They are ordinary code written down in advance, so the same confirmed symptoms always produce the same answer.

22 of 36 rules were reviewed by a physician assistant in August 2026. The rest were built from published guidelines and have not yet been reviewed by a clinician. Each rule below says which applies to it. The app says the same thing in its onboarding and on every result, under “What led to this.”

RF001 Chest pain with associated cardiac symptom

Applies when you report pain pressure tightness or squeezing in the chest; and any one of: pain spreading to arm shoulder neck or jaw; sudden cold sweat not explained by exertion or heat; shortness of breath or difficulty breathing; feeling sick to the stomach with or without vomiting.

RF002 Severe respiratory distress

Applies when you report breathlessness severe enough to break speech.

RF004 Stroke signs

Applies when you report any one of: weakness or numbness on one side of the body; drooping on one side of the face; new difficulty forming or finding words.

RF009 Severe sudden abdominal pain

RF012 Anaphylaxis with airway involvement

Applies when you report any one of: swelling of face lips tongue or throat; high pitched or squeaky sound when breathing in.

RF014 Neonatal fever

Applies when you report elevated body temperature.

Only for newborns under 28 days.

Only with a confirmed fever of 100.4°F or higher.

RF023 Signs of serious infection

Applies when you report elevated body temperature; and any one of: new confusion disorientation or lethargy versus baseline; feeling weak or wiped out all over, not one side; awareness of rapid or irregular heartbeat.

RF027 Airway involvement with voice change

Applies when you report voice suddenly hoarse, muffled or different; and any one of: swelling of face lips tongue or throat; high pitched or squeaky sound when breathing in; shortness of breath or difficulty breathing.

RF029 New leg weakness or numbness

Applies when you report any one of: new numbness or weakness in one or both legs; numbness in the inner thighs, groin, or area that would touch a saddle.

RF032 One-sided leg swelling with breathlessness

Applies when you report new swelling, tenderness or warmth in one leg but not the other; and shortness of breath or difficulty breathing.

RF033 Pain out of proportion to appearance

Applies when you report pain far worse than the area looks, or redness spreading quickly.

RF034 Cardiac symptoms without classic chest pain

Applies when you report burning in the chest or throat, often after eating; and sudden cold sweat not explained by exertion or heat.

Burning epigastric discomfort with diaphoresis is a common presentation of myocardial ischemia in women, older adults, and people with diabetes. Label changed from 'atypical' because the guideline explicitly recommends against that term, since it has been shown to make patients and clinicians underestimate severity. RF035 Flank pain with fever or instability

Applies when you report pain in the side or back below the ribs; and active vomiting; and any one of: elevated body temperature; fainting or blacking out, any duration; new confusion disorientation or lethargy versus baseline.

RF036 Vaginal bleeding with pain or instability in possible pregnancy

Applies when you report vaginal bleeding outside a normal period; and any one of: pain in the lower abdomen or pelvis; pain anywhere in the abdomen; lightheadedness vertigo or feeling faint; fainting or blacking out, any duration; feeling weak or wiped out all over, not one side.

Only when pregnancy is possible.

RF016 Suicidal ideation

Applies when you report any expression of self harm intent or wanting to not exist.

RF010 Acute testicular pain

RF021 Headache with light sensitivity and neck stiffness

Applies when you report sudden severe headache described as worst ever; and light is painful to look at or makes things worse; and neck is stiff or painful to bend forward.

RF030 New loss of bladder or bowel control

Applies when you report new inability to control urination or bowel movements, not just discomfort.

RF020 Palpitations with dizziness or syncope near miss

Applies when you report awareness of rapid or irregular heartbeat; and any one of: lightheadedness vertigo or feeling faint; fainting or blacking out, any duration.

RF024 Flank pain with persistent vomiting

Applies when you report pain in the side or back below the ribs; and active vomiting.

RF026 Vaginal bleeding in possible pregnancy

RF031 Possible acute abdominal emergency

Applies when you report pain that started near the belly button and moved to the lower right side.

RF017 Danger zone vital signs with any symptom

With any symptom, if Apple Health shows heart rate over 100, breathing rate over 20 per minute, or blood oxygen under 92%. Readings can only raise the level, never lower it, and readings taken during exercise are set aside.
